Preheat oven to 375 degrees F.

Mix all ingredients except ketchup. Form into loaf shape, place in glass baking dish. Cover with ketchup.

Bake until a meat thermometer reads at least 160 degrees F, about an hour. Remove from oven and let sit for 10 minutes to allow juices to settle before slicing.
